1 Applicable object and appointment method
  (1) The person during the self-initiated prevention period (the date of entry and the 1st-7th day after entry; the date of contact and the 1st-7th day after contact) please read the essentials carefully, then click the button "I understand" on bottom of the page to make an appointment. (online appointment only)
  (2) The person who have needs for out-of-pocket testing (not under the self-initiated prevention period) please read the essentials carefully, then click the button "I understand" on bottom of the page to make an appointment. (online appointment only)
    Note 1: Our hospital will arrange for different risk objects to be taken inspection in different inspection rooms to implement inspection flow control measures.
    Note 2: The person specified by the Central Epidemic Command Center please call administrative consultation hotline: (02) 2871-2121 ext. 86029.

2. Open online appointment time
  You can make an appointment for out-of-pocket PCR testing for COVID-19 within 13 days, and the appointment must be completed at least two working days before the testing day. (online appointment only)

3. Documents should be prepared for out-of-pocket PCR testing for COVID-19
  (If you do not take complete documents, you will not be accepted.) (Please prepare photocopy/paper document of all documents, we will not be returned for the sake of file keeping.)
  (1) Application Form for Out-of-Pocket Polymerase Chain Reaction (PCR) Testing for COVID-19.
  (2) First visit form (for the first visit patient). Download for the national and foreign national with NHI card / Download for the foreign national without NHI card
  (3) Identity card or resident certificate(original and photocopy / If you do not bring the original, you will not be accepted.). If you need an English report or go abroad, you must also bring the passport (original and photocopy/ If you do not bring the original, you will not be accepted.).
  (4) You can bring your NHI card to accelerate checking of your travel history. (non-essential)

4. Testing fee (including registration fee, diagnostic fee, testing fee and one English report.)
  (If the person during the self-initiated prevention period need to do other examinations after the physician”¦s evaluation, the relevant expenses will be charged separately.)
  (If you need the Chinese report or more than one report, please go to the medical record application counter on the first floor of Outpatient Department Building 1st within the service hours [Monday to Friday 08:00-17:20, Saturday 08:00-12:00] to take the COVID-19 testing report and inform the staff of your needs.)
  (1) Rapid testing fee is NT$4500. (the COVID-19 testing report can be taken after 17:00 on the day.)
  (2) Normal testing fee is NT$3500. (the COVID-19 testing report can be taken after 10:00 on the next day.)
  Note: Only the positive COVID-19 testing report will be shown the CT value, and the negative COVID-19 testing report will not be shown the CT value.

5. Service hours, Check-in location, and Check-in time
  (1) Service hours: Monday to Friday.
  (2) Check-in location: The 5th counter of admission (discharge) & transfer services center on the 1st floor of Chung-Cheng Building. (no need to take a number)
  (3) Check-in time: 08:30-09:30 (No waiting for being late, and inspection will be carried out in the order of registration.)
  (4) Possible time interval of sampling: 08:30-11:00 (rapid testing); 08:30-14:00 (normal testing). (If all the check-in patients have been tested, the service will be terminated early.)
  (5) Possible time interval of inspection result (report time): 14:30-15:30 (rapid testing); 14:30-17:30 (normal testing).
  (6) Time for taking report (according to the service hours of counter shown in the report receipt.): after 17:00 on the day (rapid testing); after 10:00 on the next day (normal testing).

6. Documents should be prepared for taking the COVID-19 testing report
  (1) Receipt.
  (2) The report receipt for out-of-pocket PCR testing for COVID-19. (provided by the inspection place.)
  (3) The original identity document.
    A. The subject in person: bring original identity document.
    B. The trustee: bring original identity documents of both parties, and fill in the authorization information on the report receipt.
    C. The minor's legal representative: bring the legal representative's original identity documents, and the proof of the relationship between the legal representative and the subject (the original household certificate or the subject's original identity document).

7. Location and service hours to take the COVID-19 testing report
  The COVID-19 testing report is not available on Sunday and national holidays. Please take the report according to service hours of the counter. (No waiting for being late.)
  (1) Rapid testing: The COVID-19 testing report can be taken after 17:00 on the day.
    A. On the testing day 17:00-17:20: Medical record application counter (the 8th, 9th, and 10th counter) on the first floor of Outpatient Department Building 1st. Please take a number [medical record application].
    B. On the testing day 17:30-20:00: The cashier counter on the first floor of Outpatient Department Building 3rd. Please take a number.
    C. If you do not take the COVID-19 testing report on the testing day, please take it according to service hours of the normal testing.
  (2) Normal testing: The COVID-19 testing report can be taken after 10:00 on the next day.
    A. Monday to Friday 08:00-17:20: Medical record application counter (the 8th, 9th, and 10th counter) on the first floor of Outpatient Department Building 1st. Please take a number [medical record application].
    B. Saturday 08:00-12:00: Medical record application counter (the 8th, 9th, and 10th counter) on the first floor of Outpatient Department Building 1st. Please take a number [medical record application].

8. Apply for airworthiness certificate, proof of entry, or English medical certificate
(Whether you need to apply for the airworthiness certificate, proof of entry, or English medical certificate, please refer to the official regulations of the country of entry or ask the airline by yourself.)
  (1) Applicable object: the person who receive out-of-pocket PCR testing for COVID-19 at our hospital.
  (2) Suggested inspection item: we suggest you choose normal testing because you can apply airworthiness certificate , proof of entry, or English medical certificate only after taking the COVID-19 testing report.
      ”¹Rapid testing report can be taken after 17:00 on the day, but the service hours for issuing the airworthiness certificate, proof of entry, or English medical certificate has ended. So you must go to our hospital to apply for airworthiness certificate, proof of entry, or English medical certificate on the next day.
  (3) Format of certificate
    A. Airworthiness certificate or proof of entry: please prepare blank airworthiness certificate or proof of entry yourself for the physician to fill out, then take it to cashier counter of emergency to pay the fee and seal.
    B. English medical certificate: according to issuing English medical certificate procedure of our hospital.
  (4) Fee of issuing certificate
    A. The national and foreign national with NHI card: NT$100/registration fee, NT$200/per certificate fee.
    B. The foreign national without NHI card: NT$500/registration fee, NT$340/per certificate fee.
      ”¹Because airworthiness certificate,proof of entry, or English medical certificate are different kind of certificates, they need to be paid separately.
  (5) Service hours: Monday to Saturday 10:00-12:00.
  (6) Service location: Please go to the emergency room within the service hours, and inform the staff that you want to apply for the airworthiness certificate, proof of entry, or English medical certificate.
