1. Applicable object and application/appointment method for emergency
  (I) The person under home (self) isolation/quarantine who need to go out for compassionate reasons, including visiting relatives in a critical condition, attending funerals of relatives, or dealing with other urgent issues. ”÷ Please apply to the local health department or call 1922.
  (II) Short-term business travelers. (according to regulations concerning short-term business travelers”¦ applications for shortened quarantine periods in Taiwan.) ”÷ Please apply to the local health department or call 1922.
  (III) The person under home (self) isolation/quarantine who need to go out for compassionate reasons, including visiting relatives overseas in a critical condition, attending funerals of relatives overseas, or dealing with other urgent issues, and willing to fill precaution checklist. ”÷ Please apply to the local health department or call 1922.

  (V) The person under self-health management during the management period who need to visit the sick in the hospital, including visiting relatives in a critical condition, attending funerals of relatives, or dealing with other urgent issues. ”÷ Please read the essentials carefully, then click the button "I understand" on bottom of the page to make an appointment. 2. Applicable object and appointment method for outpatient
  (I) To enter other countries for the compassionate reasons listed above.
  (II) Job requirements.
  (III) To study abroad.
  (IV) Foreign nationals, mainland Chinese, or Hong Kong and Macao residents who will depart from Taiwan.
  (V) Family members of people traveling abroad for the reasons listed above.
  (VI) Approved by the Central Epidemic Command Center.
  (VII) Other issues: the person who do not meet the above-mentioned applicable objects but have needs for out-of-pocket testing.

3. Open online appointment time
  You can make an appointment for out-of-pocket PCR testing for COVID-19 within 13 days, and the appointment must be completed at least two working days before the testing day. (online appointment only)

4. Documents should be prepared for out-of-pocket PCR testing for COVID-19
  (If you do not take complete documents, you will not be accepted.) (Please prepare photocopy/paper document of all documents, we will not be returned for the sake of file keeping.)
  (I) [Emergency/Outpatient] Application Form for Out-of-Pocket Polymerase Chain Reaction (PCR) Testing for COVID-19.
  (II) [Emergency/Outpatient] First visit form (for the first visit patient). Download for the national and foreign national with NHI card / Download for the foreign national without NHI card
  (III) [Emergency/Outpatient] Relevant documents of the reason for applying for entry such as work certificate or school attendance certificate. (in accordance with the regulation of applying out-of-Pocket PCR testing for COVID-19 from CDC, click to download)
  (IV) [Emergency] Identity card (original and photocopy / If you do not bring the original, you will not be accepted.). If you need an English report or go abroad, you must also bring the passport (original and photocopy/ If you do not bring the original, you will not be accepted.) and e-ticket or other proof of purchase (paper document).
  (V) [Outpatient] Passport (original and photocopy/ If you do not bring the original, you will not be accepted.) and e-ticket or other proof of purchase (paper document).
  (VI) [Emergency/Outpatient] People from mainland Chinese, Hong Kong and Macao need to bring Exit & Entry Permit Taiwan Republic of China.
  (VII) [Emergency/Outpatient] You can bring your NHI card to accelerate checking of your travel history. (non-essential)

5. Testing fee (including registration fee, diagnostic fee, testing fee and one Chinese or English report.)
  (If the person under self-health management need to do other examinations after the physician”¦s evaluation, the relevant expenses will be charged separately.)
  (I) Rapid testing fee is NT$7000. (the COVID-19 testing report can be taken after 17:00 on the day.)
  (II) Normal testing fee is NT$6000. (the COVID-19 testing report can be taken after 10:00 on the next day.)

6. Service time, Check-in location, and Check-in time for emergency
  Service day: Monday to Friday.
  Check-in location: 1st cashier counter of emergency.
  (I) The person under home (self) isolation/quarantine was arranged to testing at our hospital by the local health department.
    A. Check-in time: Monday to Friday 08:30-15:00. (Rapid testing is limited before 09:00.) (No waiting for being late, and inspection will be carried out in the order of registration.)
    B. Possible time interval of sampling: 08:30-11:00 (rapid testing); 08:30-18:30 (normal testing). (If all the check-in patients have been tested, the service will be terminated early.)
    C. Possible time interval of inspection result (report time): 14:30-15:30 (rapid testing); 14:30-22:00 (normal testing).
    D. Time for taking report (according to the service hours of counter shown in the report receipt.): after 17:00 on the day (rapid testing); after 10:00 on the next day (normal testing).
  (II) The person under self-health management (the 15th-21st day after entry or contact / during the management)
    A. Check-in time: Monday to Friday 08:30-09:00. (No waiting for being late, and inspection will be carried out in the order of registration.)
    B. Possible time interval of sampling: 08:30-11:00 (rapid testing); 08:30-18:30 (normal testing). (If all the check-in patients have been tested, the service will be terminated early.)
    C. Possible time interval of inspection result (report time): 14:30-15:30 (rapid testing); 14:30-22:00 (normal testing).
    D. Time for taking report (according to the service hours of counter shown in the report receipt.): after 17:00 on the day (rapid testing); after 10:00 on the next day (normal testing).

7. Service time, Check-in location, and Check-in time for outpatient
  Service time: Monday to Friday.
  Check-in location: First-visit registration counter on the first floor of Outpatient Department Building 1st. (Regardless of first visit or return visit patients, all must go to the first-visit registration counter for document review.)
  Sampling within a particular interval of time at our hospital. Please allow time to come to our hospital early to apply and complete to check-in. If you cannot complete to check-in at the inspection counter on the first floor of Outpatient Department Building 2nd , you must wait until the next session.(Even if the check-in at the inspection counter has been completed in advance, taking inspection must be carried out in sequence of the original appointment session and number, and processing in advance will not be accepted.)
  (I) Session 1/Morning/Rapid testing
    A. Acceptance time of first-visit registration counter: 07:40-09:00. (only for rapid testing)
    B. Deadline for check-in at the inspection counter: until 09:30. (please complete check-in 10 minutes in advance.)
    C. Possible time interval of sampling: 09:30-11:00. (take inspection according to the appointment number.)(Even if the check-in at the inspection counter has been completed in advance, taking inspection must be carried out in sequence of the original appointment session and number, and processing in advance will not be accepted.) (If all the check-in patients have been tested, the service will be terminated early.)
    D. Possible time interval of inspection result (report time): 14:30-15:30.
    E. Time for taking report (according to the service hours of counter shown in the report receipt.): after 17:00 on the day.
  (II) Session 2/Morning/Normal testing
    A. Acceptance time of first-visit registration counter: 09:30-10:30.
    B. Deadline for check-in at the inspection counter: until 11:00. (please complete check-in 10 minutes in advance.)
    C. Possible time interval of sampling: 11:00-14:00. (take inspection according to the appointment number.)(Even if the check-in at the inspection counter has been completed in advance, taking inspection must be carried out in sequence of the original appointment session and number, and processing in advance will not be accepted.) (If all the check-in patients have been tested, the service will be terminated early.)
    D. Possible time interval of inspection result (report time): 16:30-17:30.
    E. Time for taking report (according to the service hours of counter shown in the report receipt.): after 10:00 on the next day.
  (III) Session 3/Afternoon/Normal testing
If your appointment number of afternoon consulting room 2117 is No.1-40, please come to our hospital for getting out-of-pocket PCR testing for COVID-19 in Session 3.
    A. Acceptance time of first-visit registration counter: 11:00-14:00.
    B. Deadline for check-in at the inspection counter: until 14:30. (please complete check-in 10 minutes in advance.)
    C. Possible time interval of sampling: 14:30-18:30. (take inspection according to the appointment number.)(Even if the check-in at the inspection counter has been completed in advance, taking inspection must be carried out in sequence of the original appointment session and number, and processing in advance will not be accepted.) (If all the check-in patients have been tested, the service will be terminated early.)
    D. Possible time interval of inspection result (report time): 21:00-22:00.
    E. Time for taking report (according to the service hours of counter shown in the report receipt.): after 10:00 on the next day.
  (IV) Session 4/Afternoon/Normal testing
If your appointment number of afternoon consulting room 2117 is No.41 or later, please come to our hospital for getting out-of-pocket PCR testing for COVID-19 in Session 4.
    A. Acceptance time of first-visit registration counter: 11:00-15:30.
    B. Deadline for check-in at the inspection counter: until 16:00. (please complete check-in 10 minutes in advance.)
    C. Possible time interval of sampling: 16:00-18:30. (take inspection according to the appointment number.)(Even if the check-in at the inspection counter has been completed in advance, taking inspection must be carried out in sequence of the original appointment session and number,and processing in advance will not be accepted.) (If all the check-in patients have been tested, the service will be terminated early.)
    D. Possible time interval of inspection result (report time): 21:00-22:00.
    E. Time for taking report (according to the service hours of counter shown in the report receipt.): after 10:00 on the next day.

8. Documents should be prepared for taking the COVID-19 testing report
  (I) Outpatient/emergency receipt.
  (II) The report receipt for out-of-pocket PCR testing for COVID-19. (provided by the inspection place.)
  (III) The original identity document.
    A. The subject in person: bring original identity document.
    B. The trustee: bring original identity documents of both parties, and fill in the authorization information on the report receipt.
    C. The minor's legal representative: bring the legal representative's original identity documents, and the proof of the relationship between the legal representative and the subject (the original household certificate or the subject's original identity document).

9. Way, location, and service time to take COVID-19 testing report for emergency
  (I) Way for taking the COVID-19 testing report
    A. The person under home (self) isolation/quarantine
      (A) Authorize others to take the COVID-19 testing report.
      (B) Fill out the authorization before testing, and authorize our hospital to fax the COVID-19 testing report to the health department.
    B. The person under self-health management
      (A) Take the COVID-19 testing report in person.
      (B) Authorize others to take the COVID-19 testing report.
  (II) Location and service time to taking COVID-19 testing report
      A. Rapid testing: The COVID-19 testing report can be taken after 17:00 on the day.
      B. Normal testing: The COVID-19 testing report can be taken after 10:00 on the next day.
      C. After the available time for COVID-19 testing reports, you can take the report at emergency cashier counter all day (24 hours). Please take a number.

10. Location and service time to take COVID-19 testing report for outpatient
  The COVID-19 testing report is not available on Sunday and national holidays. Please take the report according to service time of the counter. (No waiting for being late.)
  (I) Rapid testing: The COVID-19 testing report can be taken after 17:00 on the day.
    A. On the testing day 17:00-17:20: Medical record application counter (the 9th and 10th counter) on the first floor of Outpatient Department Building 1st. Please take a number [medical record application].
    B. On the testing day 17:30-20:00: The cashier counter on the first floor of Outpatient Department Building 3rd. Please take a number.
    C. If you do not take the COVID-19 testing report on the testing day, please take it according to service time of the normal testing.
  (II) Normal testing: The COVID-19 testing report can be taken after 10:00 on the next day.
    A. Monday to Friday 08:00-17:20: Medical record application counter (the 9th and 10th counter) on the first floor of Outpatient Department Building 1st. Please take a number [medical record application].
    B. Saturday 08:00-12:00: Medical record application counter (the 9th and 10th counter) on the first floor of Outpatient Department Building 1st. Please take a number [medical record application].

11. Apply for airworthiness certificate or English medical certificate
  (I) Applicable object: the person who receive out-of-pocket PCR testing for COVID-19 at our hospital (include outpatient and emergency testing).
      ”¹The person under self-health management with the negative COVID-19 testing report can directly go to the outpatient to apply airworthiness certificate or English medical certificate.
  (II) Suggested inspection item: we suggest you choose normal testing because you can apply airworthiness certificate or English medical certificate only after taking the COVID-19 testing report.
      ”¹Rapid testing report can be taken after 17:00 on the day, but the service time for issuing the airworthiness certificate or English medical certificate has ended. So you must go to our hospital to apply for airworthiness certificate or English medical certificate on the next day.
  (III) Format of certificate
    A. Airworthiness certificate: please prepare blank airworthiness certificate yourself for the physician to fill out, then take it to project counter on the first floor of Outpatient Department Building 1st or cashier counter on the first floor of Outpatient Department Building 3rd to pay the fee and seal.
    B. English medical certificate: according to issuing English medical certificate procedure of our hospital.
  (IV) Fee of issuing certificate
    A. The national and foreign national with NHI card: NT$100/registration fee, NT$200/per certificate fee.
    B. The foreign national without NHI card: NT$500/registration fee, NT$340/per certificate fee.
      ”¹Because airworthiness certificate and English medical certificate are different kind of certificates, they need to be paid separately.
  (V) Service time and Location (out of service on outpatient closed days, Sunday, and national holidays.)
    A. Monday to Friday 11:30-12:00, 13:30-16:00
    Location: The COVID-19 inspection counter on the first floor of Outpatient Department Building 2nd.(No registration required)
    B.Saturday 08:30-12:00
    Location: Consulting Room 3602 of infectious disease clinic on the 6th floor of Outpatient Department Building 3rd. (please make an appointment in advance.)

12. Administrative consultation hotline: (02) 2871-2121 ext. 1416 (Service time: Monday to Friday 09:00-16:00; Saturday 09:00-14:00.) (out of service on outpatient closed days, Sunday, and national holidays.)
